Under federal transparency rules, insurers and hospitals now publish their negotiated rates. But they publish them as terabytes of sprawling, inconsistent machine-readable files that no consumer could ever actually read. So people still choose care blind, often paying thousands more than they needed to for the same procedure a few miles away.
